Tooth decay
Tooth decay, or just decay, infectious disease of the tooth, is a lesion of enamel, dentin,
Etiological Factors
The formation of caries is primarily due to multiple endogenous factors:
"It results from an imbalance in the bacterial flora of the biofilm present on the surface of the tooth following a sudden change in the local environment"
- Laurence Brousseau and Camille LeGoff, The determinants of caries7.
The factors determining the balance (and imbalance) of the biofilm are:
• the bacteria themselves. Every mouth has a bacterial flora. This may include several hundred species, varying from one individual to another;
• diet and frequency of absorption of inputs: diet, alcohol, cigarettes, medicines ...
• the salivary system of the host;
• the immune system of the host.
The relative importance and interactions of all these factors are not yet fully understood.
The formation of decay begins when an imbalance in the biofilm allows some bacteria to reproduce faster. Once the proportion of these populations has reached a critical threshold, the disease may develop8. The bacteria involved in the phenomenon are present in the biofilm in a healthy state. They participate in its equilibrium and are not pathogens to be destroyed preventively, hence the abandonment of the idea of a vaccine.
Some of the factors that make the biofilm conducive to cavities appear to be identified:
• genetics9;
• tobacco, alcohol;
• taking certain medicines, for example those causing a decrease in salivary secretion;
• certain changes in eating habits;
• stress: many studies have found a direct link between stress and caries10;
• the social, economic and psychosocial factors of the host11.
The evidence for sugar as a cause of decay began to be questioned in the 1990s. Studies indicate that sugar should no longer be considered as solely responsible for decay: the relationship between sugar and decay is lower today and reducing sugar consumption is no longer necessary
sYMPTOMS
The first signs may appear once the decay has reached dentin. But sometimes the pain only occurs very late, or even never. This is why it is strongly advised not to wait to have trouble to consult a dentist.
• Pain in the cold and sweat usually indicate active caries or denudation of the dental collar, which is urgently required to be treated.
• Warm or pressure pains usually signal a recurrence of decay under a filling which has evolved discreetly and caused the necrosis of the affected tooth.
• Visible damage: initially white spot (not always very visible). A brown spot (more or less dark) indicates an old carmine, remineralized, which is no longer active.
When a hole is seen in the tooth, the decay is already advanced; the tooth may need to be devitalized.
